The following must be submitted in order for any athlete to compete in any sport/athletic club at Legacy Academy, there will be no exceptions!
1) A physical signed by a physician that states your player is cleared to participate in sports. It must be less than 365 days old (if it will expire during the season a new one will need to be done when it expires in order for your player to be able to continue to participate). Download and print the form below to give to your doctor:

2) Read the Legacy Academy Athletics Handbook, SMCC Handbook, complete the Athletic Registration Form (these forms must be completed for every sport the athlete participates in):
*Payment is required prior to the first tryout/practice.
Submit payment ($100 per sport (camps/clubs prices will vary), baseball will be $120 (this includes the cost of a team cap). Your options for payment are:
- Pay in cash or check (made out to "Legacy Academy" in an envelope with your student-athlete's name on it). If you or your student-athlete is going to physically turn in your payment, please give it to Mrs. Wendel or ask the office to put it in her box.
- Pay at the machine located at the front office or over the phone with a credit card. If you pay by credit card make sure you write whom you're paying for and what sport or reason you are paying.
- Ask for an invoice to be emailed to you by Mrs. Wendel.
